「成長的天空」計劃在中學推行多年，計劃當中使用的「香港學生資料表格」已被廣泛認可為一套既客觀又有效的識別工具，協助識別中一學生的成長需要，藉以提供適切的服務。




為協助非政府機構/中學在 2005/06 學年「成長的天空」計劃結束以後能夠繼續運用此項識別工具，為有需要的中一學生提供「以校為本」的預防性及支援性活動，香港中文大學醫學院流行病學及生物統計學中心在社會福利署的委託下，發展了一套「香港學生資料表格」的電子處理系統，以電腦光碟載入問卷及識別程式，一站式處理有關資料收集、數據分析及列印分析結果的工作。在「電子處理系統」的協助之下，非政府機構/學校可因應學校及學生需要，以最合適的步伐自行處理有關識別工作。



「香港學生資料表格」

「香港學生資料表格」(HKSIF)分為兩個部份 －「香港學生資料表格－學生表格」(HKSIF-S) 及「香港學生資料表格－老師表格」(HKSIF-T)，分別由學生及老師填寫。非政府機構/學校可參考「香港學生資料表格」的分析結果，就學生的需要，為他們提供適切的服務。




電腦光碟的使用
學校可以選用以下其中一個方法，利用「香港學生資料表格」電腦光碟，輸入及分析學生資料：

  1. 使用連接內聯網系統的電腦於網上填寫
    • 學校把光碟內的程式安裝在內聯總機電腦系統之內。
    • 中一學生在家長同意及有關老師／社工協助下，透過校內連接內聯網系統的電腦，在網上填寫「香港學生資料表格－學生表格」。
    • 有關的老師透過校內連接內聯網系統的電腦，在網上填寫「香港學生資料表格-老師表格」。
    • 全部有關的學生及老師填妥「香港學生資料表格」後，使用光碟內已設定之程式進行問卷分析及列印分析結果。

  2. 使用問卷填寫
    • 學校先把光碟內的程式安裝在電腦系統內，並啟動程式分別將學生及老師問卷列印出來。
    • 中一學生在家長同意及有關老師／社工協助下，於校內填寫「香港學生資料表格－學生表格」。
    • 有關的老師填寫「香港學生資料表格－老師表格」。
    • 學校將填妥的「香港學生資料表格－學生表格」及「香港學生資料表格－老師表格」的數據逐一輸入電腦，並使用光碟內已設定之程式進行問卷分析及列印分析結果。

With the implementation of the Understanding the Adolescent Project (UAP) in secondary schools in the past years, its screening tool (Hong Kong Student Information Form-HKSIF) has been widely recognized and proven as an objective and effective means in identifying the developmental needs of Secondary One students for timely provision of appropriate services.

To facilitate Non-governmental Organisations (NGOs)/ secondary schools in making use of the screening tool for running school-based preventive and supportive programme for Secondary One students subsequent to the phasing out of the UAP in 2005/06 school year, the Centre for Epidemiology and Biostatistics, Faculty of Medicine of the Chinese University of Hong Kong under the commission of Social Welfare Department has developed an electronic version of the HKSIF, i.e., an E-screening Tool, for data entry, data classification and generation of screening results in one-go. With the E-screening Tool, NGOs /schools will be able to conduct the screening on their own and at a pace that suits the needs of schools and students.

The Hong Kong Student Information Form (HKSIF)

The Hong Kong Student Information Form comprises two parts, namely the Hong Kong Student Information Form - Student Form (HKSIF-S) and the Hong Kong Student Information Form - Teacher Form (HKSIF-T), which are to be completed by students and teachers respectively. With reference to the results generated from analysing the HKSIF, NGOs and school can provide appropriate preventive / supportive programmes / services in meeting the needs of students as identified.

Application of the HKSIF software

Schools may choose either one of the following ways for data entry and data classification with the E-screening tool:

  1. Complete the HKSIF by direct input of data to the computers with intranet connection.
    • Install the HKSIF software into the intranet server.
    • With parents' consent and assistance of teachers/ social workers, Form One students complete the HKSIF-S by direct input to the computers with intranet connection.
    • Teachers concerned complete HKSIF-T by direct input to the computers with intranet connection.
    • Upon completion of the HKSIF by all the students and teachers concerned, execute the programme of the HKSIF software for data classification and result generation.
  2. Complete the HKSIF on printouts.
    • Install the HKSIF software into the computer, execute the programme to print out the HKSIF-S and HKSIF-T.
    • With parents' consent and assistance of teachers/ social workers, Form One students complete the HKSIF-S on printouts in schools.
    • Teachers concerned complete the HKSIF-T on printouts.
    • Schools proceed to enter the data into the computer one by one based on the completed HKSIF-S and HKSIF-T, and execute the programme of the HKSIF software for data classification and result generation.
